Get started with Support at Home
in 4 easy steps

Decide on your funding option

Once you’ve decided that home care is right for you, you can use the online assessment tool on the My Aged Care website to check if you’re eligible for any government funding.

Apply for government funding

If you determine that you are eligible to receive government funding, you will then Apply for an assessment on the My Aged Care website. The assessment will take place in your home and will determine how much government funding you will receive to use on care hours.

Select your provider

Once you know the outcome of your assessment and you are assigned your funding you can then select your Support at Home provider. At HCA Home Care you will have a Business Relationship Lead who can work with you to sign you up for services and help you with the set up and onboarding process.

Start receiving care

Once you’ve selected your Support at Home provider you will work with them to arrange your first services. At HCA Home Care you will be assigned a Care Partner who will work with you to get your services set up.

What is the Support at Home Program (previously Home Care Packages)?

The Support at Home Program is the updated government funded model that replaces Home Care Packages. It provides older Australians with coordinated services that help them stay independent at home, including nursing, domestic support, personal care, social activities and transport.

How does HCA Home Care support people accessing the Support at Home Program?

HCA Home Care assists with understanding eligibility, preparing for assessments, explaining funding allocations and developing tailored care plans. Each person receives a dedicated Care Partner who coordinates services and ongoing support.

How do I apply for the Support at Home Program?

Applications start through My Aged Care, where you check eligibility and request a home-based assessment. A government assessor determines the level of funding you can access. HCA can guide you through this process and help you understand each step.

What services can I use my Support at Home funding for?

Support at Home funding can cover in home nursing, medication support, domestic assistance, personal care, social support, transport, meal preparation and allied health. HCA offers a broad range of these services so customers can build a care plan that suits their needs.

How does HCA’s self-managed home care option work?

Self-managed care allows you to coordinate your own services while HCA handles compliance, budgeting and reporting. It gives greater choice and control, often allowing funding to stretch further. We have developed unique systems to allow you to schedule and manage your services directly with your chosen supplier, view your care plan, monthly activity statements and even communicate with your dedicated Care Partner (Customer Care Manager).

What makes HCA Home Care different from other providers?

Does HCA offer 24 hour or overnight home care?

Yes. HCA can arrange overnight, extended hours or continuous support depending on your funding level and personal needs. Additional private care can be added if more hours are required.

What types of Support Workers does HCA provide?

HCA provides trained Support Workers experienced in assisting with meal preparation, social participation, personal care, transport, household tasks and daily routines. Clinical nursing staff are also available for medical needs.

What is the difference between Support at Home funding and private home care?

Support at Home is a government-subsidised program that requires an eligibility assessment and approved funding level. Private home care is paid for directly, does not require an assessment and can begin immediately. Many Healthcare Australia Home Care customers choose to combine Support at Home funding with privately paid services through HCA to access additional hours, faster support or greater flexibility in how their care is delivered.

How do I switch from my current provider to HCA Home Care?

Switching involves notifying My Aged Care of your decision, choosing a transfer date and allowing HCA to manage the onboarding. Your Care Partner ensures a smooth process, including liaising with your previous provider.

Does HCA provide dementia specific care at home?

Yes. HCA offers dementia informed support including structured routines, safety monitoring, behavioural strategies, meaningful engagement and personalised assistance to promote comfort and wellbeing.

Can HCA assist with returning home after a hospital stay?

HCA can support your safe transition home by arranging nursing care, wound care, medication support, mobility assistance, transport and short-term increased services during recovery.

Does HCA offer support with shopping, meal preparation and community access?

Yes. HCA Support Workers can help with grocery shopping, attend appointments, assist in the community, prepare meals at home and support participation in social activities that promote wellbeing.
