Understanding Support at Home

More support, more choice with the Support at Home program

The Support at Home program is designed for older Australians who need regular, ongoing support to live independently and safely at home.

Support is personalised to your needs and preferences, with funding provided based on an assessment that places you into one of eight funding classifications. These classifications reflect the level of care and services you may require, from occasional help to more complex, daily support.

Once you’ve been approved for Support at Home funding, you can choose any registered provider across Australia. Your provider will work with you to build a support plan that reflects your goals and lifestyle, giving you more choice, flexibility, and control.

At HCA Home Care, we help turn your Support at Home funding into meaningful care. From understanding what your funding covers to arranging services that fit into your week, we help bring the right support together so home continues to feel like home.

Depending on your situation, services you may receive include help with daily tasks, personal care, health-related support, home safety modifications, or short-term recovery assistance, all aimed at helping you maintain your quality of life at home.

Are you a Commonwealth Home Support Programme (CHSP) customer?

What services can you access through Support at Home?

Support at Home is designed to make it easier for older Australians to access the care they need to remain safe, well and independent at home. Services are grouped into:

Clinical care

Clinical care focuses on helping you manage your health, mobility and wellbeing at home. It may include:≈

  • Nursing
  • Occupational therapy
  • Physiotherapy
  • Podiatry
  • Allied health services

HCA Home Care can help connect you with appropriate clinical care and support services based on your requirements and your approved Support at Home plan.

Independence supports

Independence supports are designed to assist with personal care, daily routines, connection and safe participation in the community. These services can help you continue living the way you choose while receiving practical support where it matters most.

They may include:

  • Personal care
  • Medication support
  • Transport
  • Respite care
  • Social support

HCA Home Care’s Support Workers can assist with everyday personal routines such as showering, dressing, grooming and medication prompts. We can also support transport to appointments, social activities and community outings, as well as respite care to give family carers time to rest and recharge.

Everyday living supports

Everyday living supports help with practical household tasks that contribute to a safer, cleaner and more manageable home environment. These services can be especially helpful when daily chores become physically difficult, time-consuming or unsafe to manage alone.

They may include:

  • Cleaning
  • Gardening
  • Shopping
  • Meal preparation
  • Home maintenance

FAQs

What is Support at Home in aged care?

Support at Home is an Australian Government aged care program that helps older people access services to live independently at home for longer. It brings together a range of in-home care services, including clinical care, personal support and essential everyday assistance.

Did Support at Home replace Home Care Packages?

Yes. Support at Home has replaced the Home Care Packages program and the Short-Term Restorative Care program from 1 November 2025. The Commonwealth Home Support Programme is expected to transition later. For people who previously received a Home Care Package, the transition is designed to move them into Support at Home while continuing access to in-home aged care services.

What services are included in Support at Home?

Services may include nursing, allied health, personal care, transport, respite and medication support. You may also be eligible for household support such as cleaning, gardening, shopping, meal preparation and home maintenance. Additionally, your plan may include assistive technology and home modifications, with approved services guided by your aged care assessment.

Do I have to pay for Support at Home services?

Some Support at Home services may require a personal contribution, depending on the type of service you receive and your individual financial circumstances. Clinical supports are generally treated differently from independence and everyday living supports, which may have contribution arrangements. Rest assured your fees and contributions will be explained clearly before services begin.
