Eating well plays an important role in maintaining your strength, energy and overall wellbeing. But changes in appetite, health or your ability to eat can sometimes make getting the nutrition you need more difficult.
Nutrition is a Clinical Support service under Support at Home. Depending on your assessed needs, support may include dietary advice, prescribed nutrition supplements or specialised products and equipment.
At HCA Home Care, we can help coordinate approved nutrition support around your individual needs and wider care plan.
No two people need the same support. That’s why our approach starts with understanding you, not simply the services you need.
Get support based on your individual health, nutritional needs and any difficulties you may be experiencing with eating or maintaining adequate nutrition.
If your nutritional needs cannot be met through food alone, prescribed oral nutrition supplements may be recommended as part of your care.
For people who require tube feeding, prescribed nutrition products and equipment may be available based on assessed needs.
Where appropriate, qualified health professionals can provide advice and recommendations to help manage your individual nutritional needs.
Nutrition support may be available through Support at Home depending on your assessment and approved services.
An aged care assessment considers your health, day-to-day needs, and the support that could help you continue living independently.
If nutrition support is included in your approved services, we’ll help you understand what you can access and how it fits within your care plan.
Your HCA Home Care Customer Care Manager can help coordinate approved nutrition services, prescribed products or equipment around your needs.
If your health or nutritional needs change, we can work with you to review your services and help coordinate what comes next.
Nutrition support may be helpful when changes in your health, appetite or ability to eat make it harder to get the nutrition you need.
Unintentional weight loss, reduced strength or increasing frailty may be signs that your nutritional needs are not being met.
Changes in appetite, swallowing or your ability to eat can make maintaining good nutrition more difficult.
The right nutrition can be particularly important when rebuilding strength following illness, surgery or a hospital stay.
Some people may require prescribed oral supplements or enteral nutrition when their nutritional needs cannot be met through their usual diet.
