If you cant find what you’re after please don’t hesitate to contact us.
The right equipment or a practical change to your home can make a big difference to how safely and independently you manage everyday life.
Assistive Technology and Home Modifications are available through the Assistive Technology and Home Modifications (AT-HM) Scheme, which sits within Support at Home. The scheme provides separate funding for approved equipment, products and changes to your home based on your assessed needs.
At HCA Home Care, we can help you understand your approval and coordinate the assessments, equipment, suppliers and modifications you need.
The AT-HM Scheme can provide access to approved equipment and changes to your home that support your safety, accessibility and independence.
Assistive technology can include approved equipment such as walking sticks, walking frames, wheelchairs, shower chairs and toileting supports that help make everyday activities safer and easier.
Approved home modifications can include grab rails, internal and external handrails, ramps, stair lifts and other changes that make your home safer and easier to access.
Where assessed as necessary, home modifications can include changes such as bathroom redesigns or widening doorways and passages to improve accessibility.
Depending on what you need, an assessment or prescription from an appropriate health professional may be required. All home modifications under the AT-HM Scheme must be prescribed by an occupational therapist.
AT-HM funding is separate from your ongoing Support at Home quarterly budget and is based on what your aged care assessment identifies you need.
Your aged care assessment considers whether assistive technology, home modifications or both could help you live safely and independently at home.
If approved, your Notice of Decision and support plan will outline the type of AT-HM support and funding tier available to you.
Depending on the equipment or modification you need, an appropriate health professional may assess your needs and provide a prescription or recommendation.
Your HCA Home Care Customer Care Manager can help arrange approved equipment, assessments, suppliers and home modifications and guide you through the process.
AT-HM support may be useful when changes in mobility, function or accessibility are making everyday life at home more difficult.
Mobility equipment or practical changes to your home can make it easier to move around safely and maintain your independence.
The right equipment can make activities such as showering, toileting, eating or moving between positions safer and easier to manage.
Home modifications such as rails, ramps or doorway changes can improve accessibility and help your home continue to work for you.
If your mobility or functional needs change over time, an assessment can help identify equipment or modifications that better support your independence.
