Your Support at Home funding moves with you, including eligible unspent funding. Your current provider also cannot charge you an exit fee for leaving.

Yes. You have the right to change your Support at Home provider at any time and for any reason. You might switch because your needs have changed, you want different services or you’re simply not happy with your current provider.
No. Your Support at Home funding is not tied to your current provider. Your funding moves with you when you change providers, including eligible unspent funds.
No. You don’t need to complete another assessment simply because you want to change providers. Your existing Support at Home approval and classification remain in place.
No. Under Support at Home, your provider cannot charge an exit amount when you leave their care.
Your remaining Support at Home budget moves with you to your new provider. There are processes in place for your outgoing provider to finalise services and share relevant budget information with your new provider.
Yes. You’ll need to let your current provider know that you’re changing providers and agree on an exit date. It’s a good idea to speak with your new provider first so you can plan the transition and reduce the risk of disruption to your care.
A well-planned transition can help minimise disruption. HCA will work with you to understand your existing services, agree when your new services can begin and help coordinate your transition.
