Assistive Technology and Home Modifications are available through the Assistive Technology and Home Modifications (AT-HM) Scheme, which sits within Support at Home. The scheme provides separate funding for approved equipment, products and changes to your home based on your assessed needs.

What’s included with Assistive Technology & Home Modifications?

The AT-HM Scheme can provide access to approved equipment and changes to your home that support your safety, accessibility and independence.

Mobility & Everyday Equipment

Assistive technology can include approved equipment such as walking sticks, walking frames, wheelchairs, shower chairs and toileting supports that help make everyday activities safer and easier.

Home Access & Safety Modifications

Approved home modifications can include grab rails, internal and external handrails, ramps, stair lifts and other changes that make your home safer and easier to access.

More Complex Home Modifications

Where assessed as necessary, home modifications can include changes such as bathroom redesigns or widening doorways and passages to improve accessibility.

Assessments & Prescriptions

Depending on what you need, an assessment or prescription from an appropriate health professional may be required. All home modifications under the AT-HM Scheme must be prescribed by an occupational therapist.

How Can I Access Assistive Technology & Home Modifications?

AT-HM funding is separate from your ongoing Support at Home quarterly budget and is based on what your aged care assessment identifies you need.

What is the Assistive Technology and Home Modifications Scheme?

The AT-HM Scheme is part of Support at Home and provides separate funding for approved equipment and home modifications that help you live safely and independently at home. Your eligibility and funding tier are based on your assessed needs.

What can Assistive Technology & Home Modifications include?

Assistive technology can include approved mobility, bathing, toileting and other equipment that makes everyday activities easier or safer. Home modifications can include changes such as grab rails, handrails, ramps, stair lifts, bathroom modifications and doorway changes.

Is AT-HM funding separate from my Support at Home budget?

Yes. AT-HM funding is separate from your ongoing Support at Home quarterly budget and can only be used for eligible assistive technology and home modifications.

Do I need an assessment before accessing AT-HM funding?

Yes. Your need for assistive technology and home modifications is considered through your aged care assessment. Some assistive technology may also require a prescription from a suitably qualified health professional, while home modifications must be prescribed by an occupational therapist.

Yes. Once you have the appropriate approval and funding, your HCA Home Care Customer Care Manager can help coordinate approved assessments, equipment, suppliers and home modifications.
