Understanding the Restorative Care Pathway

The Restorative Care Pathway is a short-term pathway under Support at Home designed to help eligible older Australians maintain or regain function and independence.

Restorative Care Management is the Clinical Support service that provides specialist coordination for people receiving care through this pathway.

Step 1

Up to 16 Weeks

A restorative care episode can provide up to 16 consecutive weeks of intensive support based on your assessed needs and goals.

Step 2

Dedicated Funding

Each episode provides dedicated restorative care funding that is separate from any ongoing Support at Home funding you receive.

Step 3

Multidisciplinary Support

Depending on your assessed needs, your care may bring together allied health and nursing professionals to help you work towards your goals.

Step 4

Focused on Independence

Your support is built around individual goals, such as improving mobility, rebuilding strength, or getting back to everyday activities.

Who Can Access Restorative Care?

Eligibility for the Restorative Care Pathway is determined through an aged care assessment.

It may be suitable if you:

  • Need short-term, targeted support
  • Could benefit from intensive allied health and/or nursing services
  • Have goals focused on maintaining or regaining your independence
  • Are willing and able to actively participate in your restorative care

If you already receive ongoing Support at Home services, you may still be able to access the Restorative Care Pathway alongside your existing support.
